Description
The Case Manager is responsible to assist in the development, planning, coordination and administration of the activities of Utilization Review and Discharge Planning. Including but not limited to daily review of medical records to determine appropriateness and medical necessity of admission, continued hospital stay and use of ancillary services.
Functions under the general supervision of the Case Management Manager. Assumes 24 hour accountability for their care managed patients. Acts as a facilitator in planning and coordinating the ! care of patients. Communicates with and acts as a liaison betw! een physicians, staff, patients, significant others and other agencies to facilitate patient care and discharge planning. Coordinates necessary documentation to reflect the type of care rendered by physicians and hospital personnel. Serves as a patient advocate.
Qualifications
Licences/Certifications:
Registered Nurse license
Experience/Skills:
Minimum of three years nursing experience required.
Excellent communication skills and ability to work collaboratively with all members of the health care team.
Maintains a working knowledge of CMS requirements for patient stays, inpatient, observation and outpatient and medical necessity criteria for that stay and any changes needed. Works as a patient advocate facilitating clincal reviews and referrals to post discharge locations. Acts as a resource to others for discharge criteria. Completes clinical reviews and provides the review to insurance companies with follo! w up for approval of the hospital stay.
Requires rotating of weekends and holiday coverage to maintain department staffing levels.
Required Registered Nurse with 2-3 years of acute care nursing experience in a hospital setting within the last 5 years. BSN preferred. Case managment experience preferred, certified Case Manager preferred.
Training in PAS certification and InterQual reviews preferred.
Prior experience in UR or Case Management preferred
