Function under the supervision of the physicians assuming multiple clinical functio! ns involved with the care of the patients. Work in innovative Family Medicine practice with fully implemented EMR and developing Patient Centered Medical Home (PCMH) certification.Technical competenciesMEDICAL ASSESSMENT OF PATIENTUtilize information gathered from suppliers. Formulate medical diagnosis based on patient admitting status, and physical findings.
Implement plan of care. Document Quality Control and Improvement. Communicate to Physicians, their assessment through surveillance exams, injuries, treatment and procedures as deemed necessary. FORMULATE PLAN OF CAREDevelop individualized plan of care for all assigned patients.
Consistently evaluate the effectiveness of care provided to the patient. Develop appropriate goals with the patient, employer, or with the client's Workers Compensation insurance company. Update and modify the plan of care. Communicate plan of care to the patient, client, and to the Worker's Compensation claim agent.
I! MPLEMENTATION OF PATIENT CAREPerform interventions to meet hol! istic needs of patient, under the direction of the Medical Director, and under the leadership of the Client Resource Manager. Access appropriate resources i.e. lab, radiology, respiratory, pastoral care, social services and behavioral health staff. Document appropriate intervention.
Institute Occupational Health Care management. EVALUATION OF EFFECTIVENESS OF CAREInvolve patient, client, claim carrier, and appropriate team members in the evaluation process. Implement admission/discharge, transfer, or referral with the help of the Case Manager. PATIENT EDUCATIONInstitute teaching.
Communicate medical advice to patient (wellness and illness related). EFFECTIVE TEAM FUNCTIONINGUtilize appropriate resources for problem solving and communication. Keep management team and Medical Director informed of significant information. Utilize time productively.
Work with other Physicians and health care workers by assisting or delegating patient care delivery. Ac! tively participate in problem solving. PROFESSIONAL DEVELOPMENTParticipate in educational offerings, attend all mandatory inservice's, and initiate make-up sessions with the appropriate sources. Assist with orientation of new employees.
Act as a role model to those around you. Education and formal training required and be an approved Nurse Practitioner.Annual education Mandatory training programs Participate in 75% of office meetings Participate in performance improvement activities related to job or facility. Seek new methods and principles Minimum qualifications EducationMasters degree in Nursing or related field is required. LicensureCertificate from an accredited NP program required.
CPR certification required. OtherMust have good communication skills in dealing with patients, physicians, and other personnel. Must have good organizational skills. Three to four years of nursing experience preferred.
Knowledge of medical terminology preferred. C! omputer experience required. EMR preferred. Epic preferred.
