Description: Hancock Regional Hospital is currently seeking to employ RN, Home Health,Expanding in the Hamilton County & surrounding areas! right now, this vacancy will be placed in Indiana. Detailed specification about this vacancy opportunity kindly read the description below. Overview:
The RN Case Manager is responsible for the assessment, plannin! g, implementation, coordination, monitoring and evaluation of services across the continuum of care to ensure quality patient outcomes and appropriate utilization of home health care services. The RN, Case Manager will provide care to patients and their families in the Hamilton County & surrounding areas.

Responsibilities:
The RN Case Manager is responsible for directing/providing patient education and is responsible for facilitating the healthcare team towards a smooth transition between levels of care and/or discharge.

Qualifications:
Demonstrates strong clinical skills, strong manangement skills, knowledge of utilization management, PPS and third party payor reimbursement.

Current in Basic Life Support training per identified standards.

Performs admission and concurrent patient assessment related to appropriateness of the level of care. OASIS requirements, procedures performed, quality expectations and utilization! issues.

Confers with interdisciplinary team members ! in developing patient specific plan of care.

Establishes the discharge plan with the patient, physician and interdisciplinary team.

Assures accuracy, completeness of clinical documentation and communicates with physician and team to ensure outcomes are met.

Develops and implements goal-directed plan of care which is prioritized and based on specific outcome criteria.

Licensed to practice as a Registered Nurse in the State of Indiana.

Licensed driver in the state of Indiana.*CB*
