Home Care offers professionals the opportunity to develop deeper relationships with people they take care of. You can give individualized attention to your client, because you are only taking care of one person at a time. You become an integral part of the services designed to offer maximum health and independence in the home setting. You become a care partner, supporting individuals who prefer to direct their healthcare from home.

We are seeking two full time RN Case Managers to help support our growth in counties just north of Fort Wayne. Qualified applicants will hold a current RN license in the state of Indiana. We feel that it is important to live close to the clients you serve, so you must reside in one of the four counties listed above ( DeKalb, Lagrange, Noble, and Steuben) to be considered for employment.

Requirements Must have at least one year’s experience as a R! egistered Nurse Experience in home care preferred Supervisory experience preferred Must possess excellent organizational skills and be able to deliver high quality client care with minimal supervision Must be willing to rotate on call responsibilities with other nurses to ensure high quality care is available at all times
