JOB SUMMARY

Performs skilled nursing functions in accordance with federal, state and local laws and within the guidelines of his/her professional organization and agency. All functions shall be perfor! med in accordance with the established policies and practices ! and standards of care for Licensed Practical Nurse. The License Practical Nurse delivers care to clients as delegated by and under the supervision of the Registered Nurse.
JOB DUTIES

Travel to patient homes and facilities

Follow care directions as listed in patient’s plan of care

Documentation of patient care, needs and progress

Educate and instruct Aides, caregivers and family members on patient’s care

Attend team conferences

PHYSICAL AND ENVIRONMENTAL DEMANDS

Complete required work on a daily basis.

Cope with work involving ill, disabled, and dying patients.

Sit, stand, bend, lift, and move intermittently. (Must be able to lift 80-100lbs)

Move equipment and supplies necessary for job functions.

Read, speak and write English in order to fulfill job functions in an understandable way.

See/hear or use prosthetics that enable accomplishment of job.

Meet! the health requirements of the agency.

May have possible exposure to infectious diseases.

Abide by all Physical safety and bloodborne pathogen protocol to minimize illness and/or injury.

Move, support and/or transfer patients as required to perform essential functions of position.

Required Skills

Nursing skills as defined by accepted nursing standards. Effective written and verbal communication skills. Good interpersonal communication skills.

Required Experience

Graduate of an accredited school of professional nursing.

Current State License as a Licensed Practical Nurse. Must have valid drivers license, r eliable transportation and automobile insurance coverage.

Preferably one (1) year experience in an acute care setting, or equivalent experience.
