Principal Duties and Responsibilities (Essential Functions)

- Assesses patients referred by Registered Nurse Case Managers for psycho/social, economic and environmental issues as they relate to patients progress in meeting goals and or discharge plan.
- Assists Registered Nurse Case Managers in identifying and accessing appropriate community resources.
- Facilitates timely placement to ECF's, residential, psychiatric, foster, and adoptive homes when assigned.
- Provides crisis intervention and supportive services to patient and family.
- Provides psycho/social counseling when appropriate to patients and families as an intervention related to discharge planning.
- Demonstrated knowledge and skills to provide appropriate care to the clients, Neonatal through Geriatric served on the assigned service unit.
- Excellent interpersonal, problem-solving and communication skills both verbally and in writing.
- Knowledge of communit! y services.
- Ability to handle a large caseload, prior! itizes effectively, and cognitively assesses the needs for discharge planning.
- Ability to work independently and in a team environment.
- Ability to assess and interpret data about the patient's status in order to identify each patient?s age specific needs and provide the care needed by the Neonatal through Geriatric groups serviced.
- Ability to respond positively to the needs of patients and family.
- Ability to address customer concerns in a private, respectful and calm manner.
- Must be compassionate patient care orientated with an effort for timeliness and accuracy in the delivery of service.
Bachelors Social Work Required
Experience
4 years Healthcare/Medical Social Worker Required
Certification/License
- Certified Cardiac Pulm Resuscitation
- Licensed Clinical Social Work
