The incumbent develops the plan for health education and assures the patient and his/her family receives the information needed focusing on self-manageme! nt, prevention, and wellness based on the patients goals. The staff nurse provides education, management and monitoring of chronic and acute disease processes and chronic pain. She/he makes assignments and provides direction to the other members of the nursing staff. She/he serves as a preceptor and mentor to colleagues and students enhancing the professional growth, personal development and practice of others. She/he is responsible for collaborating with other services internal and external to the VA to facilitate transitions of care to effectively meet the patient's goals and identified needs. She/he demonstrates leadership by active involvement in area-based councils, committees, and task forces. Maybe be required to float to other CBOCs as staff needs arise.

WORK SCHEDULE : Tour of Duty: M-F, 8:00 am - 4:30 pm. Applicant may be required to travel between CBOCs based on staffing and workload?

The Cincinnati VA Medical center is currently seeking two (2.0) Registered Nurse - RN Care Manager positions in the CBOCs assigned to Primary Care (Clermont and Dearborn CBOCS). The staff nurse manages the care for assigned primary care provider's patients by taking responsibility for the development and implementation of the patient-driven, holistic care plans across the continuum provided by Primary Care. She/he will work closely with Patient Align Care Team (PACT) which consists of a provider(s), a RN, LPN and clerical support, other clinics, internal or external services and community agencies.
