Location: Vincennes Indiana

Description: Onward Healthcare is currently seeking to employ Physical Therapist - Long Term Care / SNF Facility right now, this vacancy will be placed in Indiana. The PTs primary responsibilities will be to evaluate resident's conditions, develop treatment plans, and help restore day-today functionality.
This particular physical therapy position at this skilled nursing facility (SNF) will be contract based and provide competitive base bay and medical benefits. Relocation may also be provided for non-local candidates.

Physical Therapist will be responsible for:
Evaluate patient’s medical history
Assess the cause(s) of the patients ailment
Measure patient’s basic motor functions
Formulate patient’s goals and develop a treatment plan
Establish a daily routine and timeline
Assess patient’s progress

Position Requirements for SNF Physical Therapist Job:
Active in-state Physical Therapy license 1 year experience preferred
