Description: Healthways is at present recruited Senior Clinician, Hospital Diabetes (RN/RD/CDE) right now, this occupation will be placed in Indiana. Works with the Program Director, physicians, Resource/Utilization/Case Management, diabetes staff and other hospital staff to achieve program objectives and enhance clinical outcomes and customer satisfaction.
Assess patient's understanding of disease process; chronic complications; standards of care and current areas requiring clinical intervention via interview and self-assessment and/or medical record and assists patient to set self-care goals to improve disease management.
Provide e! ducation and implements interventions such as medical nutritio! n therapy, self blood glucose monitoring, medication/injectables, insulin pump therapy (if certified) to improve disease management.
Explores strategies to maintain and/or reduce resource consumption; to reduce length of stay; facilitates implementation of these strategies; and to monitor results.
Reviews case variances and outcomes to improve organizational and clinical systems of care.
Facilitates case management of complex diabetes cases through Diabetes Inpatient Clinicians, Case Managers, Social Workers, and other clinicians. May function as case manager, educator, dietician or nursing coordinator as needed.
May advise on diabetes wound care as needed.
Qualifications
Must be a Registered Nurse or Registered Dietician
Certified Diabetes Educator (CDE) or CDE eligible preferred
Hospital experience required, 5+ years
