Home Health Aide (PRN)

Under the general supervision of the Nurse Manager, and functional supervision of the regi! stered nurse or licensed therapist, the home health aide performs various duties in assisting with treatment and care of home health patients. The home health aide primarily assists the home care patient with meeting personal care needs and their activities of daily living. The Home Health Aide may also serve as an extension of the therapist and R.N. for simple procedures/treatments for which they have demonstrated competence.

DUTIES

1. Responsible for providing patient care and treatment in accordance with the written assignment provided by the R.N. or therapist.

2. Appropriately assesses the patient's response to care given and reports any changes in the patient's condition or concerns to the R.N. responsible for the patient's care.

3. Demonstrates respect for patient rights by maintaining patient privacy, dignity, safety, confidentiality and right to refuse or consent to treatment.

4. Demonstrates knowledge of all safety and i! nfection control policies/procedures and adheres to same.

5. Participates in interdisciplinary team conferences as assigned.

6. Follows assignments of R.N. and/or therapists and completes all assigned care each visit/encounter.

7. Collects specimens (i.e. sputum, urine, stool) as instructed and assists patient in using proper technique in obtaining same.

8. Maintains safe and clean patient environment - all items used are returned to appropriate location; basins and BSC cleaned each visit; immediate patient area straightened.

9. Effectively communicates with other members of the health care team.

10. Demonstrates knowledge of all department/hospital policies and services and practices within them.

11. Keeps Manager informed of any unusual concerns, situations, events or conditions relative to staff, patients, families or physicians.

12. Documents all care given according to procedure.

13. Charting is neat, legible, concise and accurately reflects assignme! nt.

14. Demonstrates good understanding of basic asepsis and sterile technique to avoid infection of patient's and self or contamination of equipment and supplies.

15. Maintains professional relationship with other members of the health care team, patients and their families.

16. Remains available and accessible at all times while on duty or on-call and responds according to policy.

17. Demonstrates understanding of the disaster plans and role requirements of same.

18. Willingly floats to other units during times of need.

QUALIFICATIONS

  • Ability to read, write and follow both written and verbal instruction
  • Current CPR certification
  • Minimum 1 year experience as a Nursing Assistant in a formal setting and/or successful completion of a home health aide certification process. Associates with appropriate experience may be certified after employment following applicable state and federal law.!
