Diabetic Educator Care Manager

Evolent Health is looking for a Diabetic Educator Care Manager to be a key member of the Population Health Team at Indiana University Health Plan. This individual will play ! a critical role in executing Evolent Healthâs mission of partnering with the nationâs leading health systems in their strategic evolution to positively impact and improve the healthcare delivery system. As an Evolent employed team including nurses, social workers, registered dieticians, physicians, pharmacists, and program coordinators, you will have the opportunity to make a profound impact on the lives of people living with complex and/ or chronic conditions.
Responsibilities:
Conduct comprehensive assessments that identify behavioral, clinical, social and environmental concerns and needs of patients.
Based on this assessment, and in conjunction with the patient, the patientâs physician and other members of the population health team, work with the team to create a care plan that will address the identified needs, remove the barriers and improve the health of the patient.
Create a comprehensive inventory of local community and g! overnment resources for patients and their families and facili! tate the patientâs access to these resources.
Assesses the patientâs knowledge of their clinical condition and provides education and self-management support based on the patientâs unique learning style.
Develop teaching tools and programs to aid staff and self in providing patient and family education. Apply nursing process and education of patients and families including assessing needs of patients and families, formulating educational plans, implementing the plans and evaluating/revising as needed.
Assist with development and implementation of policies and procedures, standards of diabetes care and care plans related to diabetes education program and consistent with ADA accreditation.
Provides leadership with special programs such as support groups, weekend updates and other continuing education programs for patients.
Qualifications:
Required
Associate Degree in Nursing.
3-5 years of nursing ! experience; preferably in home health, ambulatory care, community public health, case management, coordinating care across multiple settings and with multiple providers.
Preferred
Bachelorâs Degree in Nursing.
A current, active, unrestricted nursing license in the State of Indiana
Currently a Certified Diabetes Educator or eligible to take the exam within two years and six months of hire date.
Exceptional communication skills, both written and oral, ability to positively influence others with respect and compassion; fluency in a foreign language is a plus.
Computer proficiency in MS office.
