Description: St. Vincent Health is presently looking of Nurse Practitioner - Congestive Heart Failure - Lebanon right now, this career will be placed in Indiana. This Nurse Practitioner will work in an Outpatient clinic where they will administer cardiac risk factor management and Congestive Heart Failure examination. You will also supervise exercise and chemical stress testing as well as perform Inpatient and Emergency Department consultation. Our perfect candidate will also have the ability to work with electronic medical records. Candidates with previous experience with inpatient and outpatient management of coronary artery disease, myocardial infarction, hypertensio! n, and congestive heart failure are also preferred.
ESSENTIAL FUNCTIONS:
1) Independently develops the plan of care utilizing advanced assessment, knowledge, skills, teaching, and critical thinking to optimize patient outcomes.
2) Consults and collaborates with other members of the health care team as appropriate to enhance patient outcomes across the continuum of care.
3) Counsels and educates patients, families, caregivers, health care team members in the management of health care needs.
4) Participates in periodic evaluation of care provided.
5) Administrative responsibilities may include chart documentation, billing submissions and participation in performance improvement initiatives.
6) May provide direct treatment and management of health conditions via prescriptive authority or referral to other health care providers or community resources.
7) Prescriptive authority, if applicable, directed by the sponsoring physician, as required in the Collaborative Agr! eement.
8) Ensures compliance with regulatory requirements and hospital policies.
BASIC QUALIFICATIONS:
1. Three (3) years of clinical nursing experience
2. MSN from a formalized, accredited program
3. Valid Indiana State RN license
4. Current NP Certification by ANA, National Certification Corporation, or Specialty Organization departments
5. Preferred qualifications listed above
