Provides education and inservices concerning medications to Pharmacy staff, nursing and physicians. Oversees and reviews all standing order and protocol or! der sets, assuring annual review and medical staff review. Develops and oversees content of Pharmacy and Therapeutics Committee meetings to provide a quality, cost-effective, controlled closed formulary. Develops, supports and monitors clinical programs (i.e., Medication Usage Evaluations, Pharmacokinetics, Adverse Drug Reactions, Monitoring of Renally-Eliminated Medications, IV to PO Monitoring, and Anticoagulant Monitoring). Acts as resource for Pharmacy, nursing, medical staff or other healthcare professionals concerning all aspects of pharmaceutical therapy. Perform occasional staffing for the Department of Pharmacy.
Responsibilities:
Associate is capable of delivering care/service appropriate to patients/customers. Assists in the coordination of Pharmacy education programs for Pharmacists and Technicians. Develops age-based competency education program for Pharmacists and Technicians. Develops new clinical programs and Pharmacy services. Serves as r! esource for the Pharmacy, nursing, medical staff, or other hea! lthcare professionals. Provides inservices to nursing or other ancillary departments when needed. Supports ongoing clinical programs (e.g., Medication Usage Evaluations, Kinetics, Adverse Drug Reactions/Events). Develops policies and procedures to support changes in clinical programs when necessary. Provides quality, cost-effective, individualized drug therapy information focused on appropriate drug usage and age-specific populations. Assists in the development and application of a consistent means of evaluating formulary requests/deletions. Participates in the development of a controlled formulary, reviewing drug classes on an as needed basis. Presents formulary evaluations to various committees (e.g., Pharmacy and Therapeutics Committee) when needed. Always strives to make good use of time by assisting with additional projects on a regular basis. Attends meetings regularly. Shows knowledge in using office equipment unassisted (Microsoft Office, Micromedex). Demonstrates k! nowledge of hospital and Pharmacy policies. Maintains records of continuing education for license renewal. Takes time to stop and assist anyone who requests help. Willing to work with co-workers who need help completing work before deadlines. Maintains a professional appearance and positive attitude at all times.
Qualifications:
Must be a graduate of accredited school of pharmacy and licensed in the state of Indiana. Strong knowledge of clinical monitoring and pharmacokinetic monitoring. Ability to provide statistical analysis. Knowledge of systems for obtaining published clinical information. Must be self-directed, motivated, mature and assume a broad range of responsibilities.
