The primary function of the Nurse Practitioner is to assist the Medical Director/Collaborating Physician by ensuring maintenance of high standards of resident care! by expanding access to advanced practice nursing including assessment, treatment, medication and therapy in accordance to the State of Indiana Board of Nursing Scope of Practice for Advanced Practice Nursing/Nurse Practitioner. The position also facilitates the day to day communication between front line care givers and advanced practice professionals and provides for timely, accurate and effective response to changes in resident condition.

Required Skills

Graduate of a College of Nursing and Advanced Practice Nursing/Nurse Practitioner program which meets the requirements of the State of Indiana State Board of Nursing for Advanced Practice Nursing with current license, in good standing, to practice as a registered nurse and as an advanced practice nurse/nurse practitioner in the state of Indiana. Able to prescribe medication, treatment, therapy and other functions as outlined by, and within the scope of, Advanced Practice Nursing/NP practice as determine! d by the State of Indiana Board of Nursing.

2 or more! years experience as CNP.

Able to read, write, speak and understand English. Must have excellent oral and written communication skills and must be able to create and edit reports, orders and documents using computer applications.

Ability to collaborate with inter and intra disciplinary teams

Excellent clinical and analytic skills. Strategic thought process to permit maximization of funding sources.

Highly organized, able to keep accurate and timely records of care and to maximum service plans and plan codes to ensure all care provided is reimbursable as well as necessary and appropriate.

Ability to stand and walk for long periods of time, use hands and arms with dexterity, occasionally stoop, kneel, bend or crouch. Must frequently lift, move or physically manipulate residents using proper body mechanics. Must be able to communicate clearly and effectively with residents. Visual acuity, including the ability to see at close range,! and accurately perceive color required. Requires peripheral vision, depth perception and ability to adjust focus. Must be able to tolerate moderate noise levels and discern conversation in atmosphere of background noise.

Required Experience

Practice will include those functions as within the current (at all times) Indiana State Board of Nursing Scope of Practice for Advanced Practice Nursing/Nurse Practitioners (document attached is current as of Sept 2010)

Diagnose, treat, evaluate and manage residents' acute and chronic conditions in collaboration with the Medical Director.

Obtain/perform and document medical histories, physical examinations, treatment plan.

Establish good working relationships with state ombudsmen and surveyors.

Order, perform, and interpret diagnostic studies such as routine lab tests, bone x-rays, EKGs.

Prescribe physical therapy and other rehabilitative or restorative treatment.

! Report problems to the administrator

Prescribe pharmacological! treatment for acute and chronic disease; monitor resident condition and make medication changes as needed

Provide wellness screenings and health maintenance procedures.

Counsel and educate residents and families on health behaviors, self-care skills and treatment options.

Work with the Director of Nursing and Staff Development Coordinator to provide staff in-service training as needed.

Document and code care procedures in compliance with Third Party Billing company procedures to ensure timely, accurate and comprehensive billing of services provided.

Coordinate inter-disciplinary communication on case management issues.

Assist with the coordination of transfers and discharges with other departments, hospitals, and families.

Complete management reports timely, accurately and completely.

Assess resident needs and make recommendations for improvements in the care plan.

Evaluate resident care relat! ed to individualized needs or preferences, family involvement and physicians orders.

Establish and maintain open communication throughout the department and with other departments.

Establish and maintain good relationships with referral sources, participate in facility tours and model principles of customer service to staff.

Become familiar with facility clinical metrics, participate with the team to achieve excellence

Develop and maintain cordial, effective relationship with Third Party Biller, and resolve billing issues promptly and effectively

Serve as facility On-Call person on rotating schedule

Learn to use and navigate Point Click Care

Serve as a member of the facility's Quality Assurance Committee and provide clinical support and leadership to the committee.
