Location: Warsaw Indiana

Assist nurses by scheduling and transporting consumers to and from medical and therapeutic related appointments. Provide prompt transportation, verify that proper paperwork! is accurately completed by persons served, communicate necessary medical information to physician, maintain inventory of service materials, process and file paperwork. Report physician orders to nurse, managers and DSPs. Interact with consumers for the purpose of guidance, care, and support.

Hours: 9a-4p as needed according to appointments. 20 hrs/week.

QUALIFICATIONS

Requires a H.S. diploma/GED plus one year experience in a human service field in addition to a current valid driver's license and reliable vehicle.
