Improving PCP performance by providing feedback to PCP’s on ! a regular basis in both formal written and informal means on quality and utilization data.

Supporting Evercare Health Services by providing Education for Nurse Practitioners, Physician Assistants and Care Managers.

Quality and Utilization Data Analysis and Management by leading site Clinical Quality Committee to assure compliance with CMS, and company-wide clinical performance improvement initiatives.

Serving as a partner with the Health Services Director (HSD) for medical care delivery issues related to external providers.

Enhancing growth and new business development.

Represent physician perspectives in geriatric medicine and chronic illness management to facilitate site strategic planning for new product development.
ABMS Board Certified/Board Eligible in Internal Medicine or Family Practice

ABMS Board Certified in Geriatrics and/or Hospice and Palliative Care or substantial clinical experience in geriatric practi! ce.

Minimum of 5 years of geriatric and long-term car! e experience.
Significant health care management/leadership experience preferred.
